A toddler from Utrecht suffered severe injuries on Tuesday while playing on a slide at a McDonald's restaurant in Huis ter Heide, resulting in the surgical amputation of her toe at a hospital.
Eyewitnesses alerted the media outlet Hart van Nederland to the incident after local residents noticed the screaming child and provided immediate assistance at the scene, where the victim was found bleeding heavily inside her shoe.

The Dutch Food and Consumer Product Safety Authority (NVWA) confirmed that an official report has been filed regarding the amusement equipment and has launched a formal investigation into the matter.
The victim's mother stated that her daughter cried intensely during the ordeal and demanded the immediate removal of the playground structure due to recurring safety issues.
Jolanda, a local resident who witnessed the aftermath, said: "Mijn man heeft het zwaargewonde meisje en haar moeder gelukkig kunnen helpen.
We hoorden ineens enorm gegil. Iedereen keek natuurlijk.
De schoen van het meisje moest uit, alles zat onder het bloed."
The mother mentioned that she had previously alerted the restaurant about a visible crack in the structure, which was only temporarily repaired before failing again.
"It is zo verdrietig, mijn meisje heeft heel erg veel gehuild. Ik kan het zelf nauwelijks bevatten, zo veel is er gebeurd.
Ik kom al jaren bij deze McDonald's dan heb ze in het verleden ook al eens gewaarschuwd voor deze glijbaan, omdat er een scheur in zat.

Dat wordt dan weer even gemaakt, maar vervolgens gaat het opnieuw mis.
Maar het lijkt mij wel duidelijk: dit speeltoestel moet onmiddellijk weg," said the mother of the injured girl.
A safety official confirmed that a probe has been initiated following the accident, though the timeline for the results remains undetermined.
"Ja, wij gaan onderzoek doen.
Hoe dat eruit gaat zien en wanneer we daar de uitslag van verwachten, daar kan ik nu nog geen antwoord op geven," said a spokesperson for the NVWA.
The fast-food chain stated that they contacted the family directly and emphasized that their recreational equipment undergoes regular mandatory safety inspections.
"We zijn natuurlijk ook geschrokken van wat er is gebeurd. Onze eerste aandacht ging uit naar hoe het nu met het meisje gaat.
We hebben daarom contact gehad met de moeder.

Al onze speeltoestellen worden jaarlijks gekeurd door een daartoe bevoegde keuringsinstantie, dus ook dit speeltoestel," said a spokesperson for McDonald's Nederland.
